Information for Healthcare Professionals

If you are interested in consulting your patient on the relevant multidisciplinary PTC team, please:

1. Fill out the form below.
2. Upload the patient’s relevant medical records in PDF format. You can send patient image documentation via ePACS.
3. Check the option if you want to participate in the MDT online.
4. Choose the date when you want your patient to be reviewed at the MDT.

The Client Service Coordinator will contact you within 24 hours with further details related to the MDT.
